Our team knows that it is important to pay attention to the signs and symptoms of these challenges so that patients can maintain wellness and decrease unnecessary hospitalizations.
We also know that providing home-based care for individuals is the best method in addressing and eliminating triggers that activate many of these conditions.
Due to the ongoing pandemic and because hospitals face a reduced capacity to care for these patients, this need will continue to grow. In response, we have innovated and created team- based programs full of resources to assist patients in maintaining autonomy, self-reliance, and safety.

Additional Mental and Behavioral Health Programs
A team consisting of a mental health registered nurse, social worker, psychiatric nurse practitioner, Community Health Worker, Behavioral Health Occupational Therapist create a plan to treat patients 18 and older with the diagnosis of a severe mental and/or behavioral health illness and with demonstrated poor coping/maladaptive behaviors/frequent rehospitalization.
