VNA of Ohio Hospice is a special way of caring for patients who have limited life expectancy and wish to have treatment of pain and symptoms rather than curative treatment. The goal of hospice is to improve the quality of a patient's days by offering comfort and dignity, while controlling pain and discomfort and to make every day count for the patient as well as the family.
An individualized plan of care is developed for each patient and is updated routinely as the patient's condition develops. Care is provided by a team of specially-trained hospice professionals and volunteers who focus on physical needs of the patient and of the patient's family.
Hospice is for patients with a limited life expectancy, wherever they call home. Top diagnosis include:
• All cancers
• Alzheimer's and dementia
• Lung diseases
• Cardiovascular diseases
Hospice focuses on maximizing the patient's comfort by reducing pain and addressing the physical, psychological, social and spiritual needs. Services include:
• Ongoing medical care and coordination including pain and symptom management
• 24-hour on-call crisis support services
• Patient and caregiver education
• Art and massage therapy
• Social work services and linkage to community resources
• Specialty hospice services for patients with Alzheimer's/dememtia
• Bereavement and spiritual care services for family members following their loved one's death
Care is provided by a multidisciplinary team of professionals including Hospice-certified nurses, home care aids, physicians, art and massage therapists, social workers, counselors, bereavement spiritual care counselors and volunteers who work together with family, physicians, and other caregivers to ensure comfort at the end of life.
VNA of Ohio Hospice promotes comfort and self-determination, enabling patients to participate in making decisions regarding their care. Our program is licensed and certified and offers crisis support 24 hours a day, 7 days a week.
VNA of Ohio also offers a pre-Hospice Special Care program, for patients with life-threatening illnesses who choose to continue to receive treatment for their illness.
Hospice care is a covered benefit under Medicare for patients with a prognosis of six months or less. A patient may remain in hospice care beyond six months when a physician confirms that the patient is terminally ill.
